I tried a google yesterday of your title ("Talking about a Revolution - Relic malfunction when starting this quest causes death"), and I found a lot of comments on reddit and elsewhere about a common bug, that causes the malfunction effect (the blurry vision and such) to not go away when it should, so it lasted 'forever'. During the conversation DO NOT select "Fucked in the head worse than me" dialogue option, if you choose it the issue will occur again. U gotta reload a saving before that convo.
